Bankruptcy Fee Waiver Request: Clients filing Bankruptcy who fall within certain income guidelines may need to request a fee waiver. If a fee waiver is granted, the online class is not available.

 

MORTGAGE FORMS

Third Party Authorization: This allows your CCCS counselor to speak to your mortgage company about your mortgage issues.

BAC Checklist & Expectations: This is for clients with a Bank of America (BAC) mortgage, and it provides you with a list of documents Bank of America will need in order to review your account for a possible loan modification, repayment plan, short sale, or other workout program. This also provides you with insights on problems that Bank of America frequently encounters with home retention applications, which can help you avoid those common mistakes.

Dodd-Frank Form: Most mortgage companies require this form when you apply for a home retention plan. By checking the box and signing this form, you are agreeing that you have not been convicted of mortgage fraud in the past 10 years.

4506T: Even though your mortgage company may ask you to submit tax returns with your home retention application, they still may want to independently verify the information contained in your tax returns. This form gives your mortgage company the authority to request a transcript of your tax returns from the IRS.

4506T EZ 2012Some mortgage companies require the 4506T, and some require the 4506T-EZ when applying for a loan modification or other plan.  Check with your mortgage company to see which they require.

2012 RMA_Request for Mortgage Assistance: When applying for a workout, your mortgage company will require this completed and signed worksheet. Your counselor can help you complete this form.
